| MARY LOU SCHWARTZ AWARD NOMINATION INSTRUCTIONS The Junior League of South Bend, Inc. is seeking nominations for the 2010 Mary Lou Schwartz Award. Established in 2003, this award recognizes the volunteer contributions of an outstanding woman from our community who exemplifies the successful volunteer fundraising spirit and ability demonstrated by the award’s namesake, Mary Lou Schwartz. Selection Criteria The Mary Lou Schwartz award will be presented to a woman who: Possesses unusual and exemplary fundraising talents and abilities; and Has demonstrated proven ability and achievement in specific fundraising for charitable entities (excluding political fundraising)
Guidelines 1. Nominations for the Mary Lou Schwartz award must be submitted on the attached Nomination Form. 2. You may include additional pages of supporting documents such as resume, awards and honors received. IMPORTANT: Any nomination which utilizes resumes, publications or articles in substitution for the responses to questions contained on the attached Nomination Form will not be accepted as complete. 3. Incomplete nominations will not be referred to the Board of Directors for review. 4. Please do not submit a nomination until you have discussed the nomination with the potential nominee. The nominee must agree to accept the award if selected and to be present at the Junior League of South Bend’s Holiday Ball on December 4, 2010 to receive the award.
